Axon Health Associates wants to provide the best quality psychiatric care to everyone that comes to our facilities. To become a new patient with Axon Health Associates, please follow the steps below.

Step One

Before we can schedule an appointment, a new patient questionnaire must be completed. Click the button below to fill out and submit our new patient questionnaire. If you are wanting to schedule an appointment for your child, please fill out the New Patient Questionnaire – Child form.

Step two

Once you have submitted the New Patient Questionnaire, our team will review it and reach out to you to schedule an appointment.

Step Three

Once an appointment has been scheduled, please return to our website and complete our new patient forms.
