Missed appointments or appointments cancelled less than 24 hours in advance (excluding weekends or holidays) are subject to a missed appointment fee of $30 for the first missed appointment in a rolling 12-month period.
Additional missed appointments or late cancellations within a 12-month period will be charged will be charged 100% of the planned service fee.
If you are more than 15 minutes late to your appointment, it may be considered as a missed appointment, and you may be asked to reschedule.
When changing from in-person to telehealth appointments, please notify the office staff as soon as possible prior to the appointment. If you have not received verbal or written confirmation of the change, the provider may not be aware of the change and could result in a missed appointment fee.
